Accessing HCC Services
People aged 65 and over or 50 and over, if they are Aboriginal or Torres Strait Islander, require basic support to remain living independently in their own home.
We fund most of the cost for the services provided to you, however a small contribution from you is requested.
Designed as an entry-level aged care support service with a focus on independence and client autonomy. 'My Aged Care' is central to you receiving a service and they are your first port of call in accessing a service.
You can call us directly or anyone with your consent can refer you to us - family, carers, friends or health professionals.
Once we receive the referral from 'My Aged Care', a Services Coordinator will visit with you to discuss your needs, eligibility and service availability. If you are happy with the service we offer, your Service Coordinator will work with you to develop a personalised care plan for you.
You may be eligible for a Home Care Package. Talk to your service coordinator about this.
