If you are aged 65 and under, or 50 and under if you are Aboriginal or Torres Strait Islander, and have a disability or condition that affects your ability to carry out daily living tasks, we can help.
Individualised Service
Some of the Services We Can Help You With:
Domestic Assistance
Assistance planning and preparing meals, cleaning & household chores, such as laundry.
Personal Care
Bathing, dressing & personal grooming.
Activities
Activities that help you to do things yourself and helping with your independence e.g. helping you to use the internet & develop skills & confidence to order your groceries for home delivery, rather than having a paid worker shopping for you.
Prompts and reminders
We can schedule a phone call or text message to remind you to do a specific task without you needing a support worker present in your home.
Assistance
Assistance with household correspondence, making appointments, placing reminder calls, helping to manage paperwork and bills.
Home Maintenance
Activities in your home, house or garden, that you are unable to manage yourself e.g. changing light bulbs, cleaning windows, lawn mowing and general garden maintenance.
Support
Support for you to access shopping and essential services, such as the bank and post office.
Support
Support for you to access & engage (or re-engage) with primary health services support (e.g. general practitioner, community mental health team) including and assisting you to make & maintain appointments.
Linking
Linking you to mainstream and other supports and services in your community, e.g. neighbourhood centres, local library, walking groups, craft groups, church groups & other informal support that may help you to achieve your support plan goals and outcomes. This may also include; Assisting you to identify your interests and find activities/groups of relevance in your local community, providing transport to and from the activities you want to attend, providing a support worker to assist you to participate in the activity, and/or transport support where you can’t access this through other means, e.g. public transport. Queensland Health or Taxi Subsidy Scheme.
